Medication errors after hospital discharge common

A study in Annals of Internal Medicine found that half of patients hospitalized with acute coronary syndromes or acute decompensated heart failure had "clinically important" medication errors 30 days after hospital discharge. A health-literacy–sensitive, pharmacist-delivered intervention didn’t significantly reduce the number of errors. Read more.
